• Pre-Program Documents

All students must sign and complete each of the forms below. The forms must be submitted prior to Residency I.

  • Apply for Financial Aid (U.S. citizens and residents only)

Complete the FAFSA form online at fafsa.gov for the 2020-2021 academic year. FIU’s federal school code for the FAFSA is 009635.

Create your Financial Student Aid (FSA) ID. Note, students can reuse their FSA ID every year.

    IMPORTANT

Students who have been admitted and completed their FAFSA application will be offered Unsubsidized Federal Stafford Loans. Graduate students may borrow up to a maximum of $20,500 per academic year through the Federal Direct Loan program. The actual amount a student is eligible to borrow cannot exceed the program cost of attendance, minus any other assistance received.

  • Finalize Your Admissions File

Acceptance to FIU is based on the academic information that was available when the Admissions Office reviews an application. Students that have been accepted on a conditional basis, must sign and return their conditional admission letters as soon as possible in order to complete their admission. Students will have one semester to matriculate to fully admitted status. It is important to submit official documents by the deadline listed in the admissions letter.

A student seeking in-state tuition must submit the required documents for proof of residency. Learn how to establish residency here: https://onestop.fiu.edu/student-records-myfiu/personal-records/declare-residency/

  • Submit Immunization Records

As a prerequisite to registration, FIU requires all students to comply with the immunization policy regulations from the Florida Board of Governors regarding immunity of Measles, Mumps, Rubella, Hepatitis B, and Meningitis.

Students can download all immunization forms at https://dasa.fiu.edu/student-support/wellness/immunization-insurance/. All forms must be signed and stamped by a physician (M.D. or D.O.), a nurse practitioner or a registered nurse. Forms must be emailed to immune@fiu.edu. For assistance, feel free to call Student Health Services (305) 348-2401.
