Admission to the Upper Division

To request admission into the College, please review the admission requirements and review your SASS report.  If you meet the admission requirements, then download the Change of Major Form, complete it, and email it to 4online@fiu.edu or deliver it to one of our offices.

Add Second/Third major

To add a second or third major, please review the requirements first.  If you meet the requirements, then download the Change of Major Form, complete it and deliver to one of our offices.

Add Minor

To add a minor offered by the College of Business Administration, please review the requirements first. If you meet the requirements, then download the Change of Major Form, complete it and deliver to one of our offices.

Course Equivalency

To request an equivalency evaluation for a course taken at another university, please download the Equivalency Request Form, complete it, and deliver it to one of our offices with the course syllabi attached. 

CLAST Exemption Form

To request exemption from the CLAST exam, please fill out the following form.
